This patient underwent breast augmentation in March 2018. She had Natrelle, Soft Touch, Moderate Profile, Silicone implants inserted. 365 cc on the right, 325 cc on the left. IMF approach was used. Submuscular, dual plane placement. Procedure was completely uneventful. No immediate complications. I attached pictures of her at 1 week, 1 month and 1 year after surgery. One week after surgery, she had a nontender, flat, erythematous area over the inferior aspect of her sternum. At one month, the area decreased in size, but became hyperpigmented. Then, over the course of several months, the patient developed a hypertrophic, irregular and contracted scar in this spot. She has no medical history, is not a smoker, and is not on any medications or supplements. She is of Asian descent. I have done a literature search and can't find any case reports or articles describing this. My working theory is that during dissection and division of the medial-inferior fibers of the pectoralis muscle, there may have been inadvertent de-vascularization of the area supplying the skin here, which led to ischemia, and then to permanent scarring. I would be curious to hear if anyone else has seen this, or if anyone has another theory as to the cause of this? Please share your thoughts.
